Below you will find some general information regarding our office policies

NEW PATIENTS

Welcome! We are looking forward to your visit and are thrilled to partner with you and your family. Please be sure to print and complete the online New Patient Forms and bring them with you to your first visit. If you are unable to do this, please arrive 30 minutes before your scheduled appointment to allow time to complete the required information.

APPOINTMENTS

For all your child's needs, whether feeling well or ill, we are here for you. You may schedule an appointment by calling our office during business hours. If you are unable to come for your scheduled visit, we kindly request you contact us as soon as possible so that your appointment time may be offered to another child. We also ask you notify us as early as you are able if you realize you will be arriving late to your appointment. If you are more than 15 minutes late for an appointment, it may be necessary to reschedule until another time.

NEWBORNS

Congratulations on your new baby and the growth of your family! It's an exciting time! Please remember to add your newest family member to your insurance policy within 21 days of birth (or at birth for children with CHIPS) in order to avoid disruption of insurance coverage.

DEMOGRAPHIC CHANGES

For your child to receive the best care possible, having the correct contact and insurance information is very helpful. If you have had a change in phone number, address or insurance plans, please notify us of these and arrive at least 10 minutes early in order to correctly update your records.
