Preparing for an Appointment

A few tips

Below you will find some general information to keep in mind for upcoming appointments

WHAT TO BRING

For EVERY appointment, caregivers are asked to please have photo identification, such as a driver's license, and a current copy of your child's insurance card. If someone other than a parent or guardian is accompanying your child, please be sure they have written permission with a parent's or guardian's signature allowing us to care for your child.

PAPERWORK TO HAVE

If it is your first visit with us, please arrive 30 minutes before your scheduled appointment time to complete the New Patient Forms, OR bring the completed paperwork with you. Please be sure to remember your child's immunization record. A copy of medical records, if available, from your child's previous physician or healthcare facility is extremely helpful. You can even have these records sent to us before your appointment in order for us to review them in advance.

INFORMATION TO KNOW

Knowing your child's current medications is tremendously helpful, and preparing your questions or concerns in advance is beneficial as well.

PREPARING YOUR CHILD

Most children feel less nervous when they are informed about what will happen at their appointment. You know your child best, but we usually recommend talking with them about what to expect at their upcoming visit. We want our patients to be as comfortable as possible!
